Scottish Transitions Forum

Summary

The Scottish Transitions Forum began in 2008 and currently has over 900 members representing a wide range of professionals, young people, parents and carers. Their vision is for all young people (14 to 25 years) who require additional support get the information and help they need to live the lives they want. In 2017 they published the ‘ Principles of Good Transitions 3’ which provides a framework to inform, structure and encourage the continual improvement of support for young people with additional needs between the ages of 14 and 25 who are making the transition to young adult life.

The Scottish Transitions Forum is supported by the Association for Real Change (ARC Scotland) - a charity that advances knowledge, practice and policy in health and social care for the benefit of people with learning disabilities or other additional support need; committed to people with learning disabilities, or other support needs being at the heart of their services and communities.
