The team assess the support needs of children who have a disability and their families. To be eligible for a service from the Disability Practice Team: - The disability of the child/young person must be substantial and be the primary reason for the referral. The team also works with children who meet their criteria, who are involved in the Child Protection process, are involved with the Children’s Hearing system, are Looked After or Looked After and Accommodated and who need support to transition to Health and Social Care. If a child is open to the team, the team will also work with siblings if there is an assessment of need and risk to be undertaken or they are impacted by the child's disability.
