This short and anonymous survey is to help Enquire, the  Scottish advice service for additional support for learning, develop clear and practical guidance on co-ordinated support plans (CSPs). We’re keen to understand how CSPs are working in real-life practice; what works well, what can be challenging, and where more support would help.

Your responses will be used by Enquire to help inform and improve our online advice on co-ordinated support plans (CSPs) for professionals. We are using MS Forms to gather questionnaire feedback and will retain this data for the duration of the project. No individuals will be identified in any published material.
